Source Title:—Enter the name of the source in this box. It might be a title, if the source is an unpublished document create a name or title for it but be sure that other people can identify the document.

An example of an unpublished document might be:

Unpublished Papers of Johnny Whosits

Author:—Enter the name of the person who wrote the book.

Publication Information:—Enter the name of the company that published the material, the city where the publisher resides, and the date of publication.

Call Number:—Just to the right of the repository button is a box where the material's call number is placed.

Actual Text:—The next box is where you place the information from the book or film. Enter the material just as it appears in the book or film. It might be just a few words, or it might be a complete paragraph. Remember, it is not necessary to enter every word of text, but it is important to enter every important word of text. Refer to the chapter lesson if you are not sure.

Comments:—Include in this box any special instructions on how to use the source or find it. You may want to include any calculations that you used to arrive at a date or age. If this source is a cemetery and directions are needed to get there, this would be the place to put those directions. Again this will print out in the source area of the report.
